Co-pays/Self Pay Patients

**We accept payments in the form of cash, check, money order Visa, MasterCard, Discover, and American Express.

Co-pays are due at the time of check-in. Any co-pay not collected at the time of the appointment is subject to additional fees associated with processing statements and mailing expenses.

Patients without insurance are expected to pay for services rendered at time of visit.

Insurance/Demographic Information

Insurance cards are to be presented at the time of check-in prior to being seen. If a patient does not have their insurance card(s), they will be responsible for payment until such proof of insurance is provided.

Registration/demographic information must be up-to-date with current data. If you have not been seen in our new facility, and you are an established patient, please expect to complete a new registration/insurance form. If you are a new patient, please be sure to give your insurance card and driver's license to the receptionist and complete all registration information to ensure accurate billing to your insurance company in a timely manner.

Medical Associates of Arlington participates with many popular insurance plans. Please feel free to contact us to find out if your insurance provider is accepted.

Participating Plans

Medical Associates of Arlington is not a participating provider with any HMO insurance product. It is the patient's responsibility to verify with their insurer provider that the individual clinician for a scheduled appointment is part of the insurance provider’s network. Any questions can be directed to the Practice Administrator.
